Accident Summary:

 ON 17 JUNE 1969 AIRCRAFT OF THE 128TH ASSAULT HELICOPTER COMPANY WERE ENGAGED IN TACTICAL OPERATIONS IN THE VICINITY OF COORDINATES XS907715, 9 NAUTICAL MILES SOUTH OF SAIGON, RVN. THE OPERATION ON THE MORNING OF THE ACCIDENT INVOLVED INSERTING TROOPS INTO A LANDING ZONE AT COORDINATES XS907715. THE OPERATION WAS SUPPORTED BY A LIGHT FIRE TEAM. OF THE GUNSHIPS PROVIDING SUPPORT, UH-1B HELICOPTER, SN: 63-12912 WAS FLYING LEAD AIRCRAFT OF THE LIGHT FIRE TEAM. THE LEAD AIRCRAFT WAS ON A HEADING OF APPROXIMATELY 280 DEGREES AT AN ALTITUDE OF 100 FEET AND AN AIRSPEED OF 60 TO 70 KNOTS WHEN THE AIRCRAFT COMMANDER FELT A JERK ON THE CYCLIC AND IMMEDIATELY NOTICED ENGINE RPM INCREASING RAPIDLY TO 7000 RPM. THE AIRCRAFT COMMANDER PULLED IN PITCH TRYING TO STOP THE GAIN IN RPM THUS PREVENTING AN OVERSPEED CONDITION. AFTER STOPPING THE GAIN IN RPM, WO1 WALTERS ROLLED THE THROTTLE OFF AND ENTERED AUTOROTATION. THE PRM DECREASED BELOW 5800 AND THE AIRCRAFT COMMANDER REDUCED COLLECTIVE PITCH AND FLARED THE AIRCRAFT TRYING TO REGAIN SUFFICIENT OPERATING RPM. A STEEP FLARE WAS INITIATED CAUSING THE STINGER TO STRIKE IN THE MUDDY CRUSTED RICE PADDY LEAVING A SKID MARK 29 FEET LONG. THE GROUND CONSISTED OF A PARTIALLY DRIED RICE PADDY CAPABLE OF HOLDING VERY LITTLE WEIGHT. AN ATTEMPT WAS MADE, BY LEVELING THE AIRCRAFT AND INCREASING PITCH, TO EXTEND HIS GLIDE OVER A BOMB CRATER DIRECTLY IN THE PATH OF THE AIRCRAFT. AGAIN AFT CYCLIC WAS APPLIED TRYING TO MAINTAIN A LEVEL ATTITUDE ALLOWING THE MAIN ROTOR TO FLEX COMING IN CONTACT WITH THE TAIL BOOM SEVERING IT FROM THE AIRCRAFT. THE AIRCRAFT WAS MOVING AT APPROXIMATELY 30 KNOTS UPON IMPACT WITH THE RICE PADDY. BOTH SKIDS SANK THROUGH THE CRUST INTO THE MUD BRINGING BOTH ROCKET PDS INTO CONTACT WITH THE GROUND. THE LEFT SKID, ROCKET POD AND MOUNT CAME INTO CONTACT WITH THE GROUND FIRST AND WERE TORN FROM THE AIRCRAFT. THE RIGHT SKID, ROCKET POD, AND MOUNT REMAINED INTACT LONG ENOUGH TO CAUSE THE AIRCRAFT TO TURN TO THE RIGHT. THE FORWARD MOMENTUM OF THE AIRCRAFT CAUSED THE AIRCRAFT TO TURN OVER ON ITS LEFT SIDE. THE MAIN ROTOR WAS TURNING SLOW ENOUGH THAT WHEN IT STRUCK THE GROUND IT BROKE LOOSE FROM THE MAST AND CAME TO REST LAYING AGAINST THE TOP OF THE AIRCRAFT. THE AIRCRAFT SLID 18 FEET FROM INITIAL POINT OF IMPACT TO FINAL RESTING PLACE. THE GUNNER WAS PINNED UNDER THE AIRCRAFT FOR A SHORT PERIOD OF TIME. HE WAS ABLE TO SLIP OUT FROM UNDER THE AIRCRAFT WITH THE HELP OF THE CREWCHIEF BEFORE THE AIRCRAFT SETTLED INTO THE MUD. AFTER THE COMPLETE CREW HAD GOTTEN AWAY FROM THE AIRCRAFT, THEY WERE CHECKED BY THE AIRCRAFT COMMANDER FOR SERIOUS INJURIES. THE AIRCRAFT COMMANDER THEN WENT BACK TO THE AIRCRAFT AND SHUT DOWN THE ENGINE AND ALL ELECTRICAL SWITCHES. ALL RADIOS, GUNS, AND PERSONAL EQUIPMENT WAS REMOVED FROM THE WRECKAGE AND BROUGHT TO PHU LOI, RVN ALONG WITH THE CREW. ALL MEMBERS OF THE CREW WERE CHECKED BY THE UNIT FLIGHT SURGEON AND WERE GIVEN MEDICAL RELEASES.\\
