Accident Summary:

 AIRCRAFT WAS ENGAGED IN TACTICAL OPERATIONS. THE OPERATION ON THE MORNING OF THE ACCIDENT INVOLVED INSERTING TROOPS INTO A LZ. THE OPERATION WAS SUPPORTED BY A LIGHT FIRE TEAM. OF THE GUNSHIPS PROVIDING SUPPORT, UH-1B, 63-12921 WAS FLYING WING ON THE LEAD AIRCRAFT. THE AIRCRAFT WAS ON A HEADING OF APPROXIMATELY 180 DEGREES, AT AN ALTITUDE OF 200 FT AGL AND AN AIRSPEED OF 60 TO 70 KTS WHEN THE AIRCRAFT COMMANDER NOTICED A SUDDEN DROP IN RPM. IMMEDIATELY HE LOWERED COLLECTIVE TO MAINTAIN ROTOR RPM AND STARTED A TURN TO A CLEAR AREA ON HIS RIGHT. THE N1 GAUGE READ 72%. THE AIRCRAFT COMMANDER REDUCED THE THROTTLE AND MOVED THE GOVERNOR TO THE EMERGENCY POSITION. HE THEN BEGAN TO INCREASE THROTTLE, BUT THE N1 READING DROPPED INSTEAD OF INCREASING. AT THIS TIME THE AIRCRAFT WAS APPROXIMATELY 50 FEET AGL, AND IN LINE WITH A DRY CULTIVATED FIELD. FACING INTO THE WIND, THE AIRCRAFT COMMANDER INITIATED A STEEP FLARE TO DISIPATE FORWARD SPEED. WHILE IN THIS ATTITUDE THE STINGER HIT THE GROUND, LEAVING A SKID MARK 25 FEET LONG. THE GROUND WAS FAIRLY SMOOTH AND CAPABLE OF HOLDING MODERATE WEIGHT. COLLECTIVE PITCH WAS APPLIED BUT THE AIRCRAFT STILL HIT HARD ENOUGH TO CAUSE THE MAIN ROTOR TO FLEX AND CHOP INTO THE TAILBOOM FORWARD OF THE VERTICAL FIN. THE AIRCRAFT SLID 61 FEET, COMING TO REST APPROXIMATELY 30 DEGREES TO THE RIGHT OF THE ORIGINAL TOUCHDOWN HEADING. THE CREW CHIEF NOTICED SMOKE AND FLAMES COMING FROM THE ENGINE EXHAUST AND INFORMED THE AIRCRAFT COMMANDER. THE AIRCRAFT COMMANDER TURNED OFF THE FUEL AND ALL OTHER SWITCHES. THE FIRE WAS SQUELCHED BY THE CREW CHIEF AND GUNNER WITH CO2 EXTINGUISHERS.\\

Summary: After an explosion from within the ship, control was lost and the aircraft turned over on its right side. The gunner was trapped.

War Story:
The 1st of February was a tragic one for the Tomahawks. While flying for the 1st Infantry Division, aircraft 921 had an explosion from within the ship. Control was lost and the aircraft turned over on its right side. The gunner was trapped in the wreckage and died along with four other ground troops. The three remaining crew members and three other ground troops escaped with minor injuries.
