Accident Summary:

On the day of the accident, the crew and aircraft were working out of Fire Support Base 22 at the coordinates XS831869. The flight landed in trail facing north on a rice paddy berm running north and south. The berm had ruts on either side where both skids would touchdown in the ruts with a two foot rise to the east of the rut in which the right skid was sitting. After sitting at flight idle for approximately 15 minutes the pilot brought the aircraft to operating RPM and started to make a takeoff. The pilot had noted that the aircraft was sitting low on the right side. When he started to takeoff, he put in the left cyclic but the aircraft started to roll to the right. The aircraft commander took control of the aircraft and gave full left cyclic and down collective but the aircraft still rolled to the right with the main rotor blade striking the ground and the ship coming to rest on its right side. The statement made by the crew chief does not coincide with any of the other witnesses, in that he states the aircraft was brought to a hover and that the rotor wash from the aircraft to the rear forced the tail boom to the right and the ship contacted the rise and rolled over on the right side. When the pilot pulled pitch and the aircraft started to rise from the left side this would give the effect of coming to a hover with a takeoff in a right side low attitude. WO1 Osborn states that the only skid that raised from the ground was the left skid. He also stated the aircraft moved to the right parallel to the rise without the tail boom turning to the right. It has also been stated by WO Osborn that the wind forced the aircraft to the right at a right side low attitude dragging the right skid on the ground, in doing so the right skid contacted the two foot rise causing the ship to roll over on the right side
