Accident Summary: On 12 Jul 68, UH-1D, 64-13644, was on an authorized mission in support of the 3d Brigade, 1st Inf Div. Prior to the flight the aircraft had been flown on a combat assault mission and had refueled at Di An, RVN. It was then flown to Phy Loi and was shut down for approximately 30 minutes. There were no indications of engine malfunctions and the flight was normal in all aspects. At approximately 1010 hours the aircraft departed Phu Loi on a single ship mission with the pilot at the controls. The aircraft commander was briefing his pilot on inflight emergency procedures and informed him that he would set up a simulated engine failure. The aircraft commander rolled off to 6000 RPM and the pilot reduced pitch and set up an autorotation to a large field. The instrument readings were 1500 feet indicated altitude and 80 knots indicated airspeed. At 800 feet indicated altitude the aircraft commander noticed the engine RPM was decaying and told the pilot to roll the throttle back to full power. The pilot told the aircraft commander that the throttle was on and the engine was not responding. At this point, the aircraft commander took the controls and attempted to increase the throttle but could not get the engine to respond. He then switched to guard and broadcasted a mayday. Observing what appeared to be a touchdown point unsuitable for landing, the aircraft commander extended his glide by pulling down his rotor rpm. The aircraft hit hard and slid approximately 8 feet, turning to the right about 20 degrees. The skids were sheared off and the aircraft came to rest in an upright position
