Accident Summary:

 AIRCRAFT ATTEMPTED A TAKEOFF AT CAMP HOLLOWAY AT 1305 HOUR. THE CE HAD JUST REFUELED THE AIRCRAFT WITH 900 POUNDS OF FUEL IN THE POL AREA. ON TWO PREVIOUS OCCASIONS THE CREW HAD REFUELED WITH 1050 AND 900 POUNDS RESPECTIVELY WITHOUT INCIDENT. THIS TIME HOWEVER THE AIRCRAFT WAS UNABLE TO TAKEOFF DUE TO THE RPM BLEEDING OFF CAUSED BY AN OVERWEIGHT CONDITION. THE AIRCRAFT COMMANDER ADVISED THE TOWER THAT HE WOULD RETURN TO THE SIDE AND BURN OFF FUEL. HE BURNED FUEL DOWN TO 750 POUNDS AND AGAIN CALLED FOR TAKEOFF. HE WAS CLEARED AND WAS ADVISED HE COULD USE WHATEVER PORTION OF THE RUNWAY HE NEEDED, AND THE TOWER GAVE THE DENSITY ALTITUDE OF 4620 FEET. THE AIRCRAFT COMMANDER THEN HOVERED BACK BETWEEN 500-700 FEET AND MADE A HOVER CHECK. HE MADE A TAKEOFF FROM THE GROUND AND GRADUALLY BEGAN TO CLIMB. HE HAD ABOUT THREE FEET OF ALTITUDE WHEN HE CLEARED THE NORTHEAST END OF THE RUNWAY. HIS HEADING WAS 045 DEG. HE NOTICED THE RPM BLEED TO 6500 DURING INITIAL LIFT OFF, BUT HIS PILOT MONITORED THE RPM AND STARTED THAT IT WAS HOLDING. THE AIRCRAFT COMMANDER STATED THAT HE FELT AS IF HE HAD REACHED TRANSITIONAL LIFT AND ABOUT 40 KNOTS OF AIRSPEED. AT ABOUT 25 FEET THE AIRCRAFT SEEMED TO LOOSE POWER AND THE RPM BEGAN TO BLEED MORE RAPIDLY. MR. WILKINSON LAST NOTICED THE RPM GOING THROUGH 6000. HE LOWERED TH COLLECTIVE TO BUILD RPM, BUT TO NO AVAIL. IMMEDIATELY AFTERWARDS HE HAD TO PULL PITCH IN ORDER TO AVOID A DITCH DIRECTLY IN HIS FLIGHT PATH. THE AIRCRAFT HIT THE GROUND IN A SLIGHT NOSE LOW ATTITUDE AND STRUCK TWO ENGINEER STAKES ABOUT SIX FEET TALL. THE TAIL ROTOR STRUCK THESE POLES. THE AIRCRAFT SLID INTO A DITCH WITH THE NOSE TURNING TO THE LEFT AND CRASHED INTO THE EMBANKMENT. THE AIRCRAFT FLIPPED OVER THROWING THE CREW CHIEF FROM THE AIRCRAFT. THE TAIL BOOM SEPARATED FALLED TO THE RIGHT, AND THE MAIN FUSELAGE CAME TO REST ON ITS RIGHT SIDE. THE MAIN ROTOR WAS BROKEN FROM THE MAST AND IMPACTED ABOUT 30 FEET TO THE LEFT OF THE FUSELAGE. THE ENGINE REMAINED RUNNING AND THE FUEL CELLS WERE RUPTURED, BUT NO POST CRASH FIRE RESULTED.\\
